The goals of this project are to (a) distribute the Diabetes: A Family Matter program and toolkit materials in 8 Appalachian Ohio counties, (b) increase knowledge about diabetes prevention, and (c) promote more healthy lifestyles. Evaluation is occurring about the ways community persons use the toolkit materials, toolkit activities, and this website to promote healthy lifestyles and share diabetes prevention and self-management information. The focus of the project is type 2 diabetes. Citizen action and local empowerment are used to increase the visibility of diabetes risks throughout Appalachia.

County teams of 4-5 members are from county health departments, federally qualified health centers, and other places in each county. The teams attended two all-day training workshops in October and November 2009. Teams were provided a stipend and other resources to promote county activities. Each team was then asked to recruit 8-12 volunteer SUGAR Helpers (SUGAR stands for Support to Unite Generations in the Appalachian Region) and train them using the program manuals and materials provided. A variety of methods were used to accomplish this. The teams will now be working individually and together through September 2010 to share the health and prevention messages with people in their counties. This project will evaluate the results of the team efforts and a final workshop where they will present their outcomes will occur August 11, 2010.
